The standard duration is approximately 6–12 months depending on thread material and treatment area, according to large-scale aggregate data[1]. However, the range of reports is wide, and data showing approximately half of the effect returning within 6 months has also been cited[2]. The 12–18 month figure often seen in advertising does not align with peer-reviewed research[2].
What duration is being reported?
It varies by material and treatment area. According to reports from groups handling over 110,000 Before-after cases, the average duration of effect is approximately 6–12 months. PDO threads are said to break down in 6–8 months, while PCL threads last somewhat longer, up to about 12 months.[1]. In other words, it is not a "one-and-done" procedure but one that is planned with intervals between treatments.
Where does the 12–18 month figure come from?
It is a commonly used marketing figure, but differs from research data. A prospective study of 3D-structured bidirectional cog threads states that compared to the 12–18 month marketing claims, peer-reviewed data do not align that closely.[2]. In the reports cited there, 50% of Sagging (skin laxity) returned within 6 months, and 14% returned as early as 8 weeks[2]. A report indicating that the pure lifting effect weakens by 6 months is also introduced[2].
Is the magnitude of effect shown in numbers?
There are still not many objectively measured data. For 3D-structured PDO threads, it has been noted that there are still few data in the literature measuring results objectively on a prospective basis[2]. In mid-face treatments combined with hyaluronic acid, there is a pilot study that evaluated efficacy and safety in a blinded manner to evaluators at the 1-year and 2-year timepoints[3]. When you look at what and how measurements were taken in the research, rather than just visual impression, the weight of the numbers becomes clear.
Does increasing the number of threads increase the effect?
The answer has not yet been determined. A randomized controlled trial has been conducted examining how the number of PDO threads affects the lifting effect[4], and one report explicitly stated that guidelines indicating the optimal number for each indication do not exist in current literature[5]. An analysis examining factors leading to dissatisfaction found that patients who received more than 10 threads tended to feel the effect was insufficient and were more likely to return for re-treatment[6].
How much is known about results beyond 1 year?
The data remain thin. While the aggregate data on over 110,000 cases is large in scale, because it was collected anonymously, satisfaction and objective metrics could not be obtained, and the limitations in evaluating complications and long-term outcomes are explicitly noted.[1]. A systematic review also points out the small number of Before-after cases, short observation periods, and lack of objective evaluation metrics[7]. The same analysis states that patient-side outcomes such as dissatisfaction have also not been adequately investigated to date[6].
How can you bridge this gap during your consultation?
The quickest way is to ask about the expected outcome for your case in terms of numbers and conditions. Which thread material, how many threads, in which areas will they be placed, and based on that combination, how many months are expected; and when the effect wears off, what is planned next. Confirming these three things during your consultation will allow you to make a judgment without being swayed by advertising figures.
Even if the effect appears shorter than expected, that alone does not necessarily mean failure. Since the range of reports itself is wide in this field, whether expectations can be shared beforehand will influence your satisfaction afterward.
