Thread woven in a mesh pattern is designed to expand the surface area in contact with tissue and receive the pulling force not as a single line but as a surface. This distributes force by arranging barbs in multiple directions—an extension of recent thread trends[1]. However, no clinical research comparing mesh structure itself to regular barbed thread was found in this literature review. If the material is PDO, it breaks down in 6–8 months regardless of shape change.[2][3].
What is the goal of using mesh?
To expand the grasping surface and distribute force. Thread with barbs arranged in multiple directions strengthens grip on tissue and disperses tension more evenly to reduce thread slippage—this is how a review organizing thread shapes explains it.[1]Mesh thread can be understood as expanding this principle from a single thread to a surface.
Whether this goal translates into a clinically meaningful difference has not been confirmed. No studies comparing mesh-structure thread to regular barbed thread were found in the literature obtained this time. The same review concludes that patient selection and planning matter more than thread type in determining outcomes.[1].
The material is PDO. How long until it dissolves?
6–8 months is the standard. PDO thread has a track record as suture material and is widely used due to its predictable resorption timeline within 6–8 months and its ability to stimulate collagen production.[2]A compilation of 111,948 cases in Japan also states that PDO thread breaks down in 6–8 months.[3].
Whether lifting continues after the thread dissolves is a separate issue. An expert consensus document on the use of dissolvable barbed thread states that collagen is produced and tightening occurs in the first 12 months due to PDO stimulation, yet clearly notes that further research is needed on persistence after the thread dissolves.[4].
What was measured with three-dimensional barbed thread?
Wrinkle severity and overall improvement were measured up to 6 months. A prospective study using 3D PDO barbed thread with barbs arranged at 120°, 240°, and 360° angles in 30 women with wrinkle severity grade 3 or 4 (mean age 58.0 years) performed evaluations using 3D imaging, WSRS, and GAIS at 1, 3, and 6 months.[2].
This study employed a multilayer insertion method using thicker 18G thread for lifting deep layers and finer 21G thread for superficial layers, reporting no thread exposure, dimpling, or infection in the 30 subjects.[2]However, the authors themselves noted the limitation that it was not blinded and had no comparison group.[2]It is necessary to read this keeping in mind it was not a study directly examining mesh thread.
Do dimpling and thread exposure occur?
These are known complications regardless of material or shape. A meta-analysis of Thread lift complications in the face identified five types: short-term symptoms such as bruising and swelling, skin dimpling and contour irregularities, sensory changes including numbness, infection and thread exposure, and nerve or parotid gland injury.[5]Skin dimpling and thread exposure are so representative that reports focus specifically on them.[6].
Factors associated with lower risk were using dissolvable thread and younger age.[7]Mesh shape does not prevent these. Insertion depth, layering, and thickness selection matter more than shape differences.[1].
What it can and cannot do
It can temporarily lift mild Sagging (skin laxity). Textbooks define Thread lift as a less-invasive method than incision that lifts facial and neck soft tissue by inserting barbed thread from above the skin.[8]The Japanese Society for Aesthetic Surgery guideline rates it as recommendation level 2 (weakly recommended), noting temporary improvement is possible but effects are modest and persistence is short.[9].
A new shape does not change this position. Whether the surface-supporting design extends duration remains unconfirmed by data—this is the accurate current status.
What to confirm at consultation
Three things to check: the material and shape of thread used, the layer and number of threads inserted, and the expected duration of results in your own face and its basis. Ask what reason led to choosing mesh thread and what aspect of your Sagging (skin laxity) determined that choice.
The range reachable by thread varies with the degree of Sagging (skin laxity) and skin thickness. Which thread shape suits you will be decided with your physician after examining your face at consultation.
