- Pores areClogging, sebum, sagging (skin laxity)—treatment varies depending on what makes them prominent
- Acne scars areRedness, hyperpigmentation, depression, elevation—treatment is selected by separating these
- Sagging (skin laxity) isSkin, fat, bone structure, volume—considered separately, and the appropriate method is chosen from injectables, devices, or surgery
- Wrinkles areExpression lines, dryness-related fine lines, lines that remain even at rest—treatment differs
- Spots, melasma, and redness are difficult to judge by appearance alone, and insurance-covered treatment may be prioritized if a skin disease is suspected
- Listed treatments areAll self-pay treatmentswith individual variation in cost, required sessions, and results
All treatments listed are self-pay (not covered by insurance). Pricing, required sessions, and downtime vary depending on the area treated, amount used, and skin condition. Click on the treatment name to move to a page with detailed explanation of that treatment and the latest pricing table.

Pores, comedones, roughness
We first divide "prominent pores" into four categories.
Clogging from comedones, excess sebum, vertical elongation of pores due to loss of elasticity, and depressions from acne scars each require different treatments. The goal of treatment is not to erase pores but to reduce what makes them prominent.
First, we classify which type you have
- Comedones and blackhead poresA type where roughness is felt to the touch and white comedones or blackheads are visible. We first remove the clogging and establish a keratin environment where recurrence is less likely.
- Sebum poresA type with excess shine and sebum secretion, appearing round and prominent mainly on the nose and forehead. Over-cleansing can worsen this.
- Sagging (skin laxity) poresA type where cheek pores appear vertically elongated and become less prominent when skin is gently pulled. Firmness and tightening treatment are considered rather than comedone removal.
- Acne scar irregularitiesA type where size and depth are uneven, larger than pores, and shadow appears when light is shone at an angle. This is evaluated as scarring in the "Acne and Acne Scars" section below.
Main approaches at our Fukuoka clinic
HydraGentile
A treatment that uses water flow and suction to remove existing comedones, sebum, and old keratin.
- FrequencyOnce per month
- DowntimeMinimal to temporary redness
Suitable forThose primarily concerned with comedones, blackheads, and roughness who want to improve skin texture first.
Not suitable for / alternative treatment recommendedThose for whom depressed acne scars or sagging (skin laxity) pores are the main concern. Periods with strong inflammation, wounds, or dermatitis.
Salicylic acid macrogol
Regularizes keratin and leads to a state where clogging, roughness, and recurring comedones are more easily improved.
- FrequencyEvery 3–4 weeks
- DowntimeDryness, redness, rarely skin peeling
Suitable forThose with recurring comedones or roughness, or those with comedone-predominant acne.
Not suitable for / alternative treatment recommendedThose with compromised barrier function and strong dryness, those with active dermatitis or wounds, or those wanting to directly improve depressions.
How treatment is divided
If you want to quickly remove existing clogging, HydraGentile is a candidate; if you want to improve the keratin environment where clogging recurs, salicylic acid macrogol is a candidate. For irregularities or vertically elongated pores, consider acne scar treatment or tightening treatment rather than continuing cleansing.
Acne and Acne Scars
Acne scars are evaluated by separating color and irregularities.
Redness, brown hyperpigmentation, depressed scars, and raised scars are separate conditions. If new acne is continuing, priority is first placed on suppressing inflammation to avoid creating new scars.
First, we classify which type you have
- Active acne and comedonesA state with white comedones, blackheads, or red swollen acne. If keratin treatment alone is insufficient, treatment at an insurance-covered dermatology clinic takes priority.
- Red acne scarsPrimarily post-inflammatory erythema or redness from blood vessels. Rather than first performing treatment for depressions, we confirm the cause of active acne or redness.
- Brown acne scarsPrimarily post-inflammatory hyperpigmentation. Avoid UV exposure and friction; consider treatments such as pico-toning depending on skin condition.
- Depressed acne scarsIce-pick, boxcar, rolling, and other types show varying responses depending on depth and presence of adhesions.
- Raised acne scarsHypertrophic scars or keloids are possible. We do not perform pico-fractional or JUVBERLOOK treatments carelessly and refer to medical institutions capable of scar treatment.
Approaches for depressions and skin texture
Pico-fractional
Delivers energy in a dotted pattern to promote collagen remodeling throughout the skin, smoothing shallow irregularities, pores, and fine lines.
- Frequency3–5 sessions
- DowntimeRedness, flushing, and dotted reactions for several days
Suitable forFor those with shallow irregularities distributed broadly across the face, who want to improve pores and skin texture at the same time.
Not suitable for / alternative treatment recommendedNot suitable for deep ice-pick scars, strongly adherent rolling scars, or those with active acne or dermatitis.
JUVEDERM VOLIFT
Injects PDLLA and uncross-linked hyaluronic acid to stimulate collagen production over time in localized depressions and pores.
- FrequencyOnce per month × 3 sessions
- DowntimeRedness, swelling, bruising, and temporary irregularities
Suitable forFor those targeting localized pores or shallow depressions in the cheeks, nose, or similar areas.
Not suitable for / alternative treatment recommendedNot suitable for those seeking immediate results, those with infection or strong inflammation at the injection site, or those with deep adhesive scarring as the primary concern.
Beyond the reach of treatments at this clinic
Cases where current treatments at the Fukuoka clinic have limited efficacy
Deep ice-pick scars or rolling scars with strong adhesion may be difficult to improve adequately with pico-fractional or injection treatments alone. During consultation, we confirm the scar type and do not recommend treatment if it falls outside the appropriate range.
From Dr. Hanegi: Rather than lumping acne scars together, we first identify whether the stage is stopping new acne, treating color, or treating depressions. Multiple types often coexist on the same face.
Age spots, freckles, melasma, and dullness
The most important step in spot removal is diagnosis before choosing the emission mode.
Age spots with clear borders, fine freckles, melasma spreading symmetrically left and right, and post-inflammatory hyperpigmentation from acne or irritation all appear brown but require different treatments.
First, we classify which type you have
- Age spots and frecklesDotted pigmentation with relatively clear borders. When diagnosis is confirmed, we target them with pico-spot treatment.
- MelasmaAppears symmetrically on the cheekbones, forehead, mouth area, and elsewhere with hazy borders; varies with sun exposure, friction, and hormones. Strong spot treatment may worsen it.
- Post-inflammatory hyperpigmentationBrown discoloration remaining after acne, eczema, friction, or procedures. We stop the underlying inflammation, avoid irritation, and assess progression and treatment accordingly.
- Pigmented lesions requiring dermatology consultationIf a spot grows suddenly, has irregular shape or color, repeatedly bleeds or forms scabs, or is raised, we refer for dermatology diagnosis before cosmetic laser.
Main approaches at our Fukuoka clinic
Pico-spot (single spot)
Targets age spots or freckles with clear borders one at a time with pinpoint treatment.
- Frequency1–3 sessions
- DowntimeScabbing and color changes for 1–2 weeks; possibility of post-inflammatory hyperpigmentation
Suitable forFor those with a few bothersome spots and want to target only those.
Not suitable for / alternative treatment recommendedNot suitable for melasma, broad color unevenness, or raised or irregular pigmentation without diagnosis. Those with many spots where total cost is unclear should consider unlimited sessions below.
Pico-spot unlimited sessions
Treats all spots on the face in one session without counting individual spots.
- FrequencyFull face in one session; we assess progression and decide on additional treatments
- DowntimeScabbing and color changes for 1–2 weeks; possibility of post-inflammatory hyperpigmentation
Suitable forFor those with many small spots across the face where per-spot pricing would be unclear.
Not suitable for / alternative treatment recommendedNot suitable for those with only a few bothersome spots. Even with unlimited sessions, we do not treat melasma or undiagnosed pigmented lesions.
Pico-toning
Low-power broad treatment addressing dullness, color unevenness, post-inflammatory hyperpigmentation, and melasma after diagnosis in a gradual manner.
- FrequencyOnce per month, considering 5–10 sessions
- DowntimeMild redness and dryness; rarely white spots or pigment changes
Suitable forFor broad dullness, color unevenness, multiple fine pigmented lesions, or melasma when the physician determines it is appropriate.
Not suitable for / alternative treatment recommendedNot suitable for those wanting to remove a single spot at once, those with active dermatitis, or undiagnosed pigmented lesions. We do not perform high-power treatment on melasma.
Massage Peel (PRX-T33)
A peeling treatment that addresses dullness, rough texture, and lack of firmness together. It is not a treatment to erase individual spots.
- FrequencyEvery 2 weeks × 4–5 sessions
- DowntimeRedness, stinging, dryness, and skin peeling
Suitable forFor those concerned with overall dullness, texture, and lack of firmness across the face rather than a single spot.
Not suitable for / alternative treatment recommendedNot suitable for those wanting to remove spots with clear borders, those with active dermatitis, wounds, or strong dryness.
Milano Repeel (BioRePeel)
A peeling treatment addressing dullness, rough texture, and lack of firmness on the face and body.
- FrequencyEvery 3–4 weeks × 3–4 sessions
- DowntimeRedness, dryness, and skin peeling
Suitable forFor those wanting to improve broad dullness, roughness, and lack of firmness on the face and body.
Not suitable for / alternative treatment recommendedNot suitable for those wanting to remove isolated spots, those in sensitive skin periods, or areas with inflammation or wounds.
When melasma is suspected
Strong pico-spot treatment of symmetric pigmentation with hazy borders, mistaken for age spots, may worsen it. We establish a foundation of sun protection and friction avoidance, and perform pico-toning only after diagnosis and with cautious low power. "Melasma always requires laser" is not the rule.
Lack of firmness, sagging (skin laxity), and contour
We differentiate between skin, fat, volume, and tissue descent.
Lack of skin firmness, laxity from fat, shadows from hollowness, and skin excess require different approaches—injection, HIFU, RF, or surgery.
First, we classify which type you have
- Lack of skin firmnessPrimarily fine texture and reduced elasticity. We consider skin-boosting injections.
- Laxity from fat and contourFor deep layers under the chin and jawline, HIFU is a candidate; for skin to shallow fat layer tightening, Thermage is an option. If fat volume itself is substantial, fat-dissolving injections can reduce it.
- Hollowness and volume lossFor volume deficit in the cheeks or temples, we consider hyaluronic acid. Excessive heat treatment can be counterproductive.
- Skin excess and severe descentThere are limits to changes achieved through devices and injections alone; we consider thread lifts and surgical options as well.
Main approaches at our Fukuoka clinic
Jal Pro Super Hydro
We inject uncross-linked hyaluronic acid and amino acids to improve hydration, firmness, and elasticity.
- FrequencyEvery 3–4 weeks × 3–4 sessions
- DowntimeSwelling, redness, bruising
Suitable forFor those who want to improve dryness and lack of firmness naturally without major contour changes.
Not suitable for / alternative treatment recommendedFor those seeking strong lifting or immediate contour changes, or those with significant skin excess.
HIFU (UTIMS A3)
Heat is applied in a dotted pattern at multiple depths to tighten deep-layer sagging in the submental area and jawline.
- FrequencyEvaluation every 3–6 months
- DowntimeRedness, swelling, tenderness to pressure, rarely nerve symptoms
Suitable forFor those with submental and contour laxity who want to address deep layers without surgery.
Not suitable for / alternative treatment recommendedFor those with minimal fat who hollow easily, those whose only concern is surface dryness or fine lines, or those with severe ptosis.
Thermage
High-frequency energy broadly heats the dermis to shallow fat layer, targeting tightening and firmness improvement at the mouth corners, cheeks, and jawline.
- FrequencyEvery 2–3 months
- DowntimeFlushing, redness, swelling
Suitable forFor those concerned about skin laxity or mouth-corner sagging who want to minimize pain and downtime.
Not suitable for / alternative treatment recommendedFor those with hollow cheeks and minimal fat, or those with significant skin excess seeking surgical-level changes.
FatX Core (Fat-dissolving injection)
A medication containing deoxycholic acid is injected into the fat layer to reduce fat cells, lightening submental and jawline sagging.
- FrequencyMultiple sessions every 2–4 weeks
- DowntimeSwelling, bruising, warmth, induration (swelling peaks at 2–3 days)
Suitable forFor those with fat-related submental or jawline sagging who want to reduce fat itself without surgery.
Not suitable for / alternative treatment recommendedFor those whose sagging or ptosis is primarily due to skin laxity, those with hollow cheeks and minimal fat, or those seeking significant fat reduction at once (we recommend liposuction).
Hyaluronic acid injection
Restores volume to temples and cheeks, correcting shadows and contours caused by hollowness.
- OngoingDepends on formulation and location
- DowntimeSwelling, bruising, rarely vascular occlusion
Suitable forFor those whose volume deficit or hollow areas create a sagging appearance.
Not suitable for / alternative treatment recommendedFor those whose sagging is caused only by skin laxity or fat weight, or those expecting major lifting from injection alone.
Dryness, radiance, skin quality
"Dryness" is not always caused by water deficit alone.
We distinguish temporary dryness, impaired barrier function, age-related loss of water retention, and inflammation-related dryness. Injection treatments and needle-free introduction treatments differ in depth of penetration and expected results.
Main approaches at our Fukuoka clinic
JUVEDERM VOLIFT XC
Hyaluronic acid is injected into the dermis to improve hydration, smoothness, and fine lines from dryness.
- FrequencyMaintenance treatment as needed based on condition
- DowntimeSwelling, bruising, temporary unevenness
Suitable forFor those who want to improve dryness and fine texture while reducing the frequency of visits.
Not suitable for / alternative treatment recommendedFor those seeking strong lifting or volume augmentation, or those with inflammation or infection at the injection site.
Moist Shot (Radiant Skin Injection)
Using hyaluronic acid and PDRN, this treatment aims to comprehensively improve skin quality including dryness, radiance, and skin texture.
- FrequencyOnce monthly for 3–4 sessions, then maintenance
- DowntimeUnevenness, redness, bruising from immediately after to 2 days post-treatment
Suitable forFor those who want to address not only dryness but also dullness, skin texture, and skin recovery capacity all together.
Not suitable for / alternative treatment recommendedFor those seeking immediate facial contour changes, those with suspected allergies to fish-derived components, or those wishing to avoid injection treatments.
Dermacio (Electroporation)
A needle-free maintenance treatment that uses electrical pulses to deliver cosmetic ingredients into the skin.
- FrequencyOnce per month
- DowntimeEssentially none
Suitable forFor those who prefer to avoid needles, those who want to gently address dryness and dullness, or those seeking post-procedure care following other treatments.
Not suitable for / alternative treatment recommendedFor those seeking results comparable to injection treatments or improvement of deep hollows and strong sagging (skin laxity).
Pluryal Densify
Polynucleotide-based preparation is injected to improve hydration, firmness, and skin quality.
- FrequencyEvery 3–4 weeks, 3 sessions
- DowntimeSwelling, redness, bruising
Suitable forFor those who want to naturally improve dryness, reduced elasticity, and fine texture, or those who wish to consult about the neck and décolletage area.
Not suitable for / alternative treatment recommendedFor those seeking treatment of redness itself (such as rosacea or capillary dilation) or those with active dermatitis or infection.
Fine lines and expression lines
Assessment examines whether they appear with movement or remain at rest.
Treatment differs depending on the type: lines from facial muscle movement, fine lines from dryness, lines etched by UV exposure and aging, or grooves from volume loss.
Main approaches at our Fukuoka clinic
Botox
Softens expression lines that appear with facial muscle movement, such as in the glabella, forehead, and crow's feet.
- OngoingApproximately 3–4 months
- DowntimeBruising, asymmetry, heaviness of the eyebrows or eyelids depending on the area treated
Suitable forFor those with expression lines that primarily appear when smiling, frowning, or raising the forehead.
Not suitable for / alternative treatment recommendedFor those with deep grooves that remain at rest, fine lines from dryness, or primarily volume deficiency.
Jart+ Young Eye
An injection treatment customized for the thin skin around the eyes to naturally address fine lines from dryness and lack of firmness.
- FrequencyOnce a month, 3–4 sessions
- DowntimeSwelling, redness, bruising
Suitable forThose with fine dryness lines and lack of firmness around the eyes.
Not suitable for / alternative treatment recommendedThose whose primary cause is orbital fat bulging, deep hollowing, or facial muscle movement.
Fine lines across the entire face that persist even without expression
If dryness and loss of hydration are central, we consider Boraite XC or skin-building injection; if deep grooves are due to volume loss, hyaluronic acid injection is an option. We do not treat all wrinkles with a single procedure—we identify the cause in each area.
Redness, sensitive skin, and uneven tone
We do not diagnose redness simply as 'sensitive skin.'
Post-acne inflammation, rosacea or capillaries, dermatitis, or barrier dysfunction from dryness each require different treatment based on the underlying cause.
First, we classify which type you have
- Redness after acneWe first suppress active acne and inflammation. If blood-vessel treatment for redness is needed, we will guide you to an appropriate medical facility.
- Flushing, capillary visibility, or suspected rosaceaIf redness worsens with bathing, alcohol, temperature changes, visible capillaries, or irritation, we prioritize dermatologic diagnosis before considering injection therapy.
- Dryness and barrier dysfunctionIf stinging occurs from over-cleansing or irritation, we first reduce irritation and focus on hydration. Once stabilized, we consider gentle introduction therapy.
- Active dermatitisIf itching, peeling, exudate, or sudden worsening is present, we do not perform cosmetic procedures and instead prioritize insurance-covered dermatology treatment.
Beyond the reach of treatments at this clinic
What Fukuoka clinic can offer
We first use VISIA and examination to distinguish brown discoloration from redness based on pigmentation and record progression. Redness accompanying dryness or barrier dysfunction can be addressed with Dermasio as a supplementary option once dermatitis has settled. We may also propose treatments such as Plureal for skin quality decline, but these do not directly treat rosacea or capillary dilation. For persistent flushing, visible capillaries, or suspected rosacea, we will refer you to dermatology or facilities offering vascular laser or IPL.
From Director Hane: Rather than uniformly recommending 'inflammation-suppressing injections' for red skin, we first confirm whether the primary factor is vascular, dermatitis, post-acne, or dryness. Choosing not to treat or recommending dermatology consultation first is also part of our diagnosis.

Skin imaging diagnosis with VISIA
Fukuoka clinic uses VISIA skin imaging equipment. We record sun spots, hidden spots, redness, pores, and skin texture under consistent conditions to assist in treatment selection and progress comparison. VISIA does not confirm a diagnosis on its own; we combine imaging with physician examination.
Risks and side effects
Any procedure carries potential risks. We explain these at consultation, from the most frequent to the most serious; below are key points to understand during your decision-making.
Injections in general (hyaluronic acid, Juvederm, Jartpro, Plureal, MoistShot)
Bruising, swelling, redness, pain, temporary unevenness, infection, allergy, hardening or nodules, etc. Risks vary by product and injection site. With hyaluronic acid fillers, extremely rarely vascular occlusion can cause skin or vision injury.
Botulinum toxin
Bruising at injection site, head heaviness. Efficacy varies individually; asymmetry may occur. Eyelid or brow drop can occur but reverses as the effect wears off.
Peeling (salicylic acid, massage peel, Milano Repeel)
Redness, stinging, dryness, and rarely peeling. Post-procedure sun exposure may cause pigmentation.
PicoSure (spot, toning, fractional)
Redness, flushing, dryness, petechiae, scabbing, post-inflammatory pigmentation, depigmentation, etc. Improper treatment of melasma may worsen it. Downtime varies by mode and power.
HydraGentle and Dermasio
Temporary redness, stinging, dryness, irritation, etc. Treatment may not be possible in areas with dermatitis, wounds, or severe inflammation.
Fat-dissolving injection (FatX Core)
Swelling, bruising, redness, pain, warmth, or hardening at the injection site. Swelling typically peaks at 2–3 days and resolves over about a week. Efficacy and required sessions vary individually based on fat volume. Asymmetry may occur.
HIFU and Thermage
Heat-related redness, stinging, and swelling. Rarely, heat may affect nerves, causing temporary numbness or difficulty moving the mouth corners. Excessive power can reduce fat and cause cheek hollowing.
Common to all procedures
Results vary individually; expected changes may not occur. Treatment using unapproved drugs falls outside the scope of the Medical Device and Drug Adverse Reaction Relief System.
