Septal Extension
Features
What is a septal extension?
When it comes to septal extension, many people probably have an image of a witch-like nose or an arrow nose. It is indeed possible to aim for such a nose with a septal extension, but because the angle can be freely extended upward or downward, it does not necessarily turn into such a shape. Septal extension is actually suited to those, common among Asians including Japanese, whose noses are short or whose cartilage itself is soft. Even if a cartilage graft is performed on such people, good results cannot be obtained because the foundation is soft. It is effective to firmly build the foundation with a septal extension and then perform a cartilage graft afterward.
Septal cartilage
Taking advantage of being in the same surgical field, it is also possible to harvest septal cartilage. In Japanese people, there are individual differences in the size of the septal cartilage due to structure, so it may be necessary to use it together with ear cartilage or rib cartilage, or with donor cartilage.
Ear cartilage
Ear cartilage is cartilage frequently used in septal extension. A sufficient amount of cartilage can be harvested, but because the ear has its inherent curvature, in many cases it requires effort such as stacking two pieces or using it together with septal cartilage. When harvesting ear cartilage, it is mainly approached and harvested from the back of the auricle. When harvested from the back, it has the advantage of being able to hide the wound on the back.
Rib cartilage
For those who have a surgical history and have harvested ear cartilage in the past, or when large, strong cartilage is needed, rib cartilage harvesting is chosen. Because the harvesting is highly invasive, it must be performed under general anesthesia. A 3-4cm incision is made on the inner side under the breast to harvest it, so it has the drawback of leaving a faint scar. However, for those who want a major change, or whose nose is extremely short, it is the most suitable material.
