Aqua facial injection (a treatment that finely injects medicament into shallow skin layers) changes what it excels at and what side effects tend to occur, depending on the injection method and formula. A fine needle is easier to place at the intended depth and suits shallow concerns such as fine lines; cannula (a blunt-tipped thin tube) has been reported to cause less bruising[1]. Hand injection and machine injection have not been found in the scope of our search to have been compared in a controlled study.
Needle injection versus cannula: what is the difference?
Needle injection is said to more easily target a specific skin layer and suits fine lines and surface-level skin concerns.[1]Cannula requires only one entry point per side of the face and more easily spreads medicament widely and evenly.[1].
In a study of 20 people who received one injection via cannula, bruising occurred in 10% and temporary swelling in 15%.[1]Needle injection methods have been reported to cause bruising in 20–30%, which is more than the cannula rate.[1].
However, this study had a small sample size and was a retrospective review. It did not compare the two methods in the same individuals.[1].
Hand injection versus machine injection: which is better?
Within the scope of our search, we found no controlled studies comparing hand injection and machine injection. We cannot yet say which is superior.
What matters is the skill of the operator. It affects the final result and safety. When a practitioner experienced in injecting hyaluronic acid into skin performs it correctly, recovery is said to be faster and side effects fewer.[2].
It has been noted that in China, as the multi-needle hyaluronic acid injection method has become widespread, reports of complications have also increased.[3]Whether by machine or by hand, it is important to confirm who is injecting and at what depth.
How does the formula affect the result?
Formulas center on hyaluronic acid, with some containing added amino acids, vitamins, and other ingredients.[1][4]A review of 13 studies examining hyaluronic acid injection into skin showed improvements in hydration, firmness, brightness, skin texture, and elasticity.[2].
In formulas with hyaluronic acid plus four types of amino acids, randomized controlled trials (studies comparing groups assigned by randomization) were conducted at multiple facilities, and no serious side effects were observed.[4].
However, the trial report itself notes that large-scale research on treatments such as aqua facial injection remains limited.[4]It is unclear how much difference the formula makes to efficacy.
What side effects should I watch for?
Common ones are mild effects such as bruising and temporary swelling.[1]In trials of amino acid-containing formulas, pigmentation (browning of skin) occurred in 4 cases and allergic reaction in 1 case, all of which resolved.[4].
Rarely, when hyaluronic acid enters a blood vessel, serious complications such as tissue necrosis or vision loss have been reported.[5]Japanese cosmetic surgery guidelines also emphasize attention to this risk.[5].
How do I choose an injection method that suits me?
The guideline is your concern location and how much bruising you can tolerate. For fine lines or surface texture concerns, the needle method, which can target a specific depth, has been gaining attention.[1]If you want to improve the face broadly or prefer to avoid bruising as much as possible, cannula is also an option.[1].
Comparing formulas based on what research exists for that product and whether an explanation of how many injections are planned is provided will make selection clearer.
What should I confirm during consultation?
Which injection method and formula are suitable cannot be determined without examining skin thickness and the depth of your concern.
During consultation, confirm with your physician which method will be used (needle or cannula), which product will be used and at what depth, and how bruising or swelling will be managed if it occurs.
