Many cannot take the first step out of fear of downtime. However, downtime is not an "unknown"—it can be predicted and controlled based on medical data. In this article, using global medical paper data, wefaceliftthoroughly and clearly explain the real downtime course and risks of, and measures to speed post-op recovery.
1. Why Facelift Downtime Occurs
There is a clear medical reason why swelling and bruising occur after facelift surgery. Facial tissue is composed of five layers from the surface inward: "skin," "subcutaneous fat," "SMAS (superficial musculoaponeurotic system)," "the space of retaining ligaments and nerves," and "periosteum and deep fascia." A facelift is not merely a surgery that pulls the surface skin.
Fundamental Lifting via the SMAS (Superficial Musculoaponeurotic System) Method
In the now-mainstream "SMAS method," the approach is from the fascial (SMAS) layer deep beneath the skin. The SMAS is a firm tissue enveloping the facial muscles of expression, and it has the major characteristic of being more resistant to stretching than skin and able to maintain a long-term lift effect.
To lift this SMAS and fix it in the appropriate position, it is necessary to undermine (detach) the skin and subcutaneous fat, and to handle the "retaining ligaments" that tether facial tissue to the bone. Because such extensive manipulation reaching the deep tissues of the face is performed, swelling and bruising (downtime) inevitably occur post-op as part of the tissue-repair process.
In fact, the length of downtime may differ by nearly twofold depending on the technique.The traditional surgery that lifts only the skin (skin-only method) tends to recover faster since there is no deep manipulation, but it has drawbacks such as widened scars from excessive skin tension and quick relapse. On the other hand, techniques that manipulate the SMAS have longer initial downtime, but because they reposition the deep tissue, they yield more natural and longer-lasting results.
2. The Post-Op Course Timeline
The course during downtime can, to some extent, be medically predicted as a schedule. Here, based on research by Warren et al. and knowledge about the standard post-op course, we explain the post-op timeline.
~24 hours post-op (caution period)
From right after surgery to the first 24 hours is the period requiring the most caution. During this time, the risk of "hematoma (blood pooling under the skin)" is highest. Since a hematoma causes strong swelling and pain, complete rest is required from the day of surgery through the next day to avoid raising blood pressure.
~3 days post-op (peak swelling)
Days 2–3 post-op are the peak of facial swelling and bruising. Due to the effects of the anesthetic fluid and tissue inflammation, the whole face is in a swollen state. Also, due to residual local anesthesia, it is very common during this period to temporarily feel odd sensations in the nerves (difficulty moving or dulled sensation), but this is a normal response.
1–2 weeks post-op
Around the one-week mark, major swelling begins to gradually subside. The discoloration from bruising (changing from bluish-purple to yellow) mostly also settles by around two weeks to a level that can be sufficiently covered with makeup.
3 months to 6 months post-op (recovery of motor nerves)
Even after visible swelling subsides, tissue repair continues inside the body. Even if there is temporary difficulty in facial expression or mild motor-nerve damage, it has generally been confirmed to recover naturally within 3–4 months post-op.
1 year post-op (full recovery of sensation)
For skin sensation (the tingling or dullness when touched) to fully return to its original state generally takes up to about 12 months. Understand that downtime has different schedules for "appearance recovery" and "sensation recovery."
3. Complication Rates and Recovery Periods Shown by Research Data
For those who feel anxious about the risks during downtime, we introduce the latest large-scale data.
"The 'Average' Downtime Shown by 3,400-Case Data"
In a 2024 study by Stein et al. using the American Board of Plastic Surgery (ABPS) database, statistical data on a total of 3,400 facelift cases over 15 years (average age 61, 91% female) were analyzed (Stein et al. 2024).
Over 84% of Patients Had Zero Complications
According to this large-scale research data, 84.3–84.5% of all patients recovered smoothly without experiencing any complications (adverse effects) post-op (Stein et al. 2024). This number shows that a facelift, if performed in an appropriate setting, is a very safe surgery.
Specific Complication Rates
The incidence of major post-op troubles is as follows, remaining at a very low level.
- Hematoma (blood pooling under the skin): 2.5–2.8% (about 3% overall) (Stein et al. 2024). This is the most likely complication, but it recovers with appropriate treatment (bedside aspiration or reoperation).
- Skin trouble (skin necrosis, etc.): 0.9–1.4% (about 1% overall).
- Infection: 0.8–0.9% (about 1% overall). It can be sufficiently controlled with prescribed antibiotics.
The proportion requiring reoperation was about 9% overall, the majority being scar revision or adjustment of slight asymmetry.
4. "When Can I Go Back to Work?" The Real Schedule for Returning to Daily Life
Considering the post-op course, the answer to "when can I work or go out" depends on the nature of the work and individual tolerance. Vigorous physical labor raises hematoma risk and should be avoided for several weeks, but for desk work, many people return 1–2 weeks later once major swelling has settled.

A facelift involving incisions does indeed have longer downtime compared with thread lifts or injections that recover in a few days. However, that this "rest period" has commensurate, definite value has been proven by objective research using AI (artificial intelligence).
The Overwhelming Rejuvenation Effect Proven by AI
In a 2021 study by Chen, Gibstein et al., pre- and post-op (one year later) photos of 105 facelift patients were objectively subjected to "facial age analysis" using four cutting-edge AIs (neural networks) (Chen et al. 2021).
The results revealed clear differences in the apparent-age rejuvenation effect depending on the technique.
Furthermore, the same study demonstrated that combining "fat grafting (an average of 17cc)" during surgery improves facial volume loss,yielding an additional 2.1 years of rejuvenation (a total of nearly 8 years younger). As a result, patient satisfaction with the outcome (FACE-Q score) is also reported to rise dramatically.
By getting through about two weeks of downtime, you can turn back the clock more than five years. This is the greatest appeal of a facelift.
5. What You Can Do Before and After Surgery to Minimize Downtime
The length and severity of downtime can be greatly changed not only by surgical skill but also by how the patient spends the time before and after surgery. Based on research by Small et al. (2015) on complication countermeasures for standard facelifts, we explain the important points.
① Complete smoking cessation for 4 weeks before and 4 weeks after surgery
Cigarettes are downtime's biggest enemy. Nicotine strongly constricts blood vessels, preventing oxygen and nutrients from reaching the skin (tissue hypoxia). According to Small et al.'s research,smokers have a 12.5-fold higher risk of skin necrosis than nonsmokers. To make the surgery succeed and speed wound healing, be sure to quit smoking for 4 weeks before and after surgery.
② Stopping supplements or certain medications (2–3 weeks before surgery)
Even those considered good for health,supplementsthere are some that cause bleeding at the time of surgery and raise the risk of hematoma (prolonged bruising). In particular, the following ingredients have a blood-thinning effect (inhibiting platelet function), so intake must be stopped 2–3 weeks before surgery.
- Garlic extract
- Ginger
- Vitamin E
- Fish oil (Omega-3, etc.)
- Glucosamine
- Green tea
- Painkillers such as aspirin / NSAIDs
③ Thorough post-op blood pressure control and rest
The "hematoma" that most worsens downtime is triggered by a sharp rise in blood pressure after surgery. When the effect of adrenaline in the local anesthesia wears off, blood pressure tends to rebound upward. Post-op, it is ideal to keep systolic blood pressure (the upper number) below 140 mmHg. To do so, it is essential to take painkillers rather than endure pain, suppress nausea with medication if present, and stay relaxed.
④ Restricting salt and fluids
To prevent the face from swelling up puffily (edema) after surgery, dietary care is also effective. By limiting excessive intake of salt and fluids, you can prevent excess fluid from pooling in tissues and make swelling subside faster.
