However, the nasal tip has a structure where cartilage tissue supports soft tissue, with skin covering the surface.
In East Asians, including Japanese, the cartilage tissue that provides this support is often small and weak, while conversely the soft tissue and skin tend to be thick.
This thickness is one cause of bulbous noses, but in such cases, even if the weak inner cartilage tissue is sutured and modified, it is obscured by the thick overlying soft tissue and skin, resulting in limited external visibility and weaker results from nasal tip refinement.
Additionally, when attempting to narrow the nose by suturing during nasal tip refinement, the nose may tilt upward or lose height due to the alar cartilage structure; therefore, combination with auricular cartilage or septal extension is often necessary to strengthen the cartilage itself.

The roundness of the nasal tip is determined primarily by the shape and spacing of the alar cartilage (lateral crus). By modifying the cartilage shape through trimming and suturing (suture technique), we eliminate the roundness of the nasal tip and create a refined nasal tip. The amount of tissue removed and the strength of suturing affect the final shape, so careful planning is necessary.
Combination with cartilage grafting to refine height as well
When nasal tip refinement alone offers limited height changes, combining with auricular cartilage grafting or the Strasbourg technique allows us to refine both the narrowness and height of the nasal tip simultaneously. We propose technique combinations tailored to each patient's condition.
Choose between Closed and Open technique
There is the "Closed technique" (endonasal incision) which does not require external skin incision, and the "Open technique" which makes a small incision in the nasal columella for a wider surgical field. The technique is selected based on the degree of deformity and the necessary range of operation.

Will nasal tip refinement alone change the height of the nasal tip?
Nasal tip refinement (cartilage trimming and suturing) alone will not significantly change nasal tip height; the primary goal is improvement of shape (roundness and narrowness). When height changes are also desired, we often combine auricular cartilage grafting, Strasbourg technique, or septal extension. At your consultation, we will propose the technique best suited to your wishes.
