What is Juvederm: The dermal regeneration mechanism of PDRN

Juvederm is a skin booster injection treatment with PDRN (polydeoxyribonucleotide) as its main component. PDRN is a mixture of low-molecular DNA fragments purified from salmon testis that acts on adenosine A2A receptors to promote anti-inflammatory effects, tissue repair, and collagen neogenesis, as demonstrated in multiple peer-reviewed papers.

According to a PDRN pharmacology review published in Frontiers in Pharmacology (2017; doi: 10.3389/fphar.2017.00224), PDRN is a deoxyribonucleotide mixture with a molecular weight of 50–1,500 KDa, manufactured through a purification process that excludes active proteins capable of triggering immune responses. The paper reports three major activities—tissue repair, anti-ischemic, and anti-inflammatory—suggesting applications in regenerative medicine.

Scientific evidence: Action on skin collagen

Research in Molecular Medicine Reports (2023; doi: 10.3892/mmr.2023.13035) demonstrated that PDRN has distinct effects on the ERK signaling pathway in human skin fibroblasts and keratinocytes, playing a role in collagen accumulation and regulation of anti-inflammatory cytokines. Additionally, a study in the International Journal of Molecular Sciences (2016; doi: 10.3390/ijms17091448) reported that PDRN suppresses melanin production and may contribute to improvement of pigmentation associated with photoaging.

Furthermore, a review in Pharmaceuticals (2021; doi: 10.3390/ph14111103) clarifies that PDRN promotes the skin healing cascade through activation of fibroblasts, keratinocytes, and endothelial cells, improving reduced collagen synthesis, impaired angiogenesis, and oxidative stress. This evidence underpins its use as an aesthetic skin booster treatment.

Procedure flow and post-treatment course

On the day of treatment, cleansing and facial wash are performed first, followed by application of anesthetic cream (topical anesthesia) as needed. After approximately 20–30 minutes of anesthesia, the PDRN preparation is injected into the dermal layer using a fine needle (approximately 32G). Injection sites are selected according to treatment goals—cheeks, forehead, periocular area, neck, etc.—and carefully distributed to achieve uniform skin quality improvement. The total procedure time, including anesthesia, is approximately 60 minutes.

Downtime and post-operative precautions

After injection, temporary redness, swelling, and pinpoint bruising may occur. These typically subside within several days to one week. Makeup is possible from the day after treatment, but on the treatment day itself, avoid strong stimuli (sauna, alcohol consumption, intense exercise). Facial cleansing is permitted from the evening of the treatment day onward, but care should be taken not to rub the treated area. Effects vary by individual; typically, multiple sessions (every 3–4 weeks) are needed to achieve noticeable results.

Expected effects and limitations

Juvederm aims to improve skin firmness, uniform texture, enhanced radiance, and reduced appearance of fine lines. However, the degree and duration of effects vary considerably among individuals. For deep wrinkles or skin laxity, single treatment efficacy is limited; consult with your physician about combining with other treatments such as hyaluronic acid fillers or Thread lift. Regular maintenance injections (every 3–6 months) are recommended in some cases to sustain results.
