Facial redness and post-inflammatory erythema are not merely surface-level skin problems, but involve complex changes in skin blood vessels and inflammatory reactions in the deeper layers of skin. Medical research has shown that behind facial redness and pigmentary disorders lies increased blood vessel distribution in the skin and elevated levels of a protein called vascular endothelial growth factor (VEGF) (Iranmanesh et al. 2021). When VEGF increases, new blood vessels are excessively formed under the skin, or existing vessels dilate, and as a result, a red color becomes visible through the skin surface.

This is because IPL light is strongly absorbed by melanin pigment in the epidermis; when applied to patients with naturally darker skin (Fitzpatrick type IV or higher), excessive heat is generated in the epidermis before light reaches the target blood vessels, increasing the risk of burns or post-inflammatory hyperpigmentation (PIH).

MNRF has been proven in rigorous randomized controlled trials to demonstrate excellent results not only for facial redness (erythema) but also for acne scars and enlarged pores simultaneously. In a clinical trial conducted in Korea (Cho et al. 2012), 30 patients (12 male, 18 female, ages 16–45) underwent full-face treatment using an FRM (Fractional Radiofrequency Microneedle) device set to a depth of 1.5 mm. As a result, by 8 weeks following just 2 treatment sessions,73.3% of patients showed improved acne scar gradeand70% of patients showed improvement in enlarged pores(Cho et al. 2012).

Furthermore, in this study, confocal microscopy image analysis was performed as an objective evaluation, and it was statistically significantly demonstrated (p < .001) that the area of facial pores decreased by 58.7% after treatment (Cho et al. 2012). In another study (Min et al. 2015), half of the face was treated with FMR and conventional bipolar RF for comparison, and as a result, FMR reduced inflammatory acne lesions by 80%, non-inflammatory lesions by 65%, and demonstrated that it was highly effective even against acne-related post-inflammatory erythema (Min et al. 2016). MNRF induces collagen production through an increase in TGF-beta1 and reconstructs (remodels) the structure of the dermis itself, making it a treatment that can comprehensively address multiple concerns such as redness, unevenness, and pores (Min et al. 2015).

"Inserting needles and applying electrical current"—hearing only this explanation would make anyone feel apprehensive. But when you look at the data, the story changes.

According to a systematic review and meta-analysis (Foppiani et al. 2025) covering 21 studies involving 540 patients, microneedling treatment has been proven to be a very safe option for facial rejuvenation. The most common side effect reported was "transient erythema (temporary redness)," with an occurrence rate of just 6.77% (Foppiani et al. 2025).

Other side effects show even lower numbers. Petechial hemorrhage and edema (swelling) were 2.07%, scaling (skin peeling) was 1.65%, burning sensation was 1.5%, dry skin was 0.55%, temporary hyperpigmentation or persistent needle marks were just 0.5%, and pruritus (itching) was 0.41% (Foppiani et al. 2025). Additionally, another systematic review (Sitohang et al. 2021) analyzed 9 randomized controlled trials (a total of 341 participants) and confirmed that microneedling has significantly shorter downtime compared to chemical peeling and ablative laser treatment, with no serious side effects reported (Sitohang et al. 2021).

Pain and duration of redness: Real patient experience

"I understand the numbers. But how painful is it actually?"—data answers this question as well.

In the aforementioned study by Cho et al. (2012), regarding pain during FRM treatment, 60% of patients reported "mild" 20% answered "no pain"and in total,80% of patients found the pain during treatment to be within acceptable limits, while "moderate" pain was 13.3% and "severe" pain was 6.7% (Cho et al. 2012). Only 16.7% of patients experienced pain lasting more than 3 days after treatment. Additionally, the average duration of visible erythema (redness) from treatment was7.8 ± 2.6 days, and all of these resolved naturally without any sequelae (Cho et al. 2012).

In contrast, light and laser treatments such as IPL and PDL carry risks of blistering, depigmentation (hypopigmentation), and prolonged post-inflammatory hyperpigmentation (PIH) if settings are incorrect. Particularly when other pigmentation abnormalities such as melasma are present, cases have been reported where laser heat causes symptom rebound and worsening (Iranmanesh et al. 2021). Therefore, for patients who wish to minimize downtime while avoiding the risk of side effects, data supports that MNRF (such as Potenza) is an extremely excellent option.

Patient satisfaction and improved quality of life (QOL)

High concordance between objective data and subjective satisfaction

Ultimately, this is the most important number.At 12 weeks post-treatment, 86.7% of patients answered "satisfied" or "very satisfied."

In contrast, only 3.3% answered "dissatisfied" and 0% answered "very dissatisfied" (Cho et al. 2012).

Interestingly, in this study, while statistically significant changes were not observed in transepidermal water loss (TEWL) measuring skin barrier function or sebum measurements, skin surface roughness temporarily increased slightly 2–4 weeks after treatment, thenshowed marked smoothness improvement by week 8(Cho et al. 2012). Additionally, dermal density measured by ultrasound showed a significant increase at week 8, objectively proving that the skin structure is being rebuilt from deep within (Cho et al. 2012). This reconstruction of collagen and elastin at the dermal level not only reduces facial redness but directly leads to pore tightening and improved skin firmness, generating high patient satisfaction.
