This patient came to us with concerns such as: ・The philtrum appears long ・The nasal tip looks rounded ・They wanted both the front and side profile to look more refined In this case, we combined: ・Septal extension ・Cartilage grafting ・Tip plasty ・Nasal implant ・Alar reduction ・Nasolabial fold / paranasal augmentation to improve the overall facial balance. When someone wants the philtrum to look shorter, philtrum shortening surgery is not always necessary. By adjusting the position and direction of the nasal tip, the projection of the columella, and the balance of the nasolabial angle, the philtrum can appear shorter as part of the overall facial impression. In this case, septal extension was used to adjust the nasal tip position, while refining the rounded tip and adding height. As a result, the balance with the mouth area improved, creating a more three-dimensional side profile. From the front view as well, the rounded appearance of the nasal tip is reduced, giving the overall face a more refined impression.
